It sounds like someone asked "how are you", they said "I am fine", and then the first one did not really believe that. So they said: I am fine, because I say so / I am fine, I tell you I am.
I feel fine because I'm talking to you right now.
I think it's an answer in response to someone asking him/her "how are you?"
