Ok, I'll assume you need the point-slope form.
y - y1 = m(x - x1)
in your case y1 = -9, x1 = -2, m = 1/2
just substitute
y + 9 = (1/2)(x + 2)
you can rearrange that solution to get another form, but you're not saying anything about that.
