Do you wait for an answer besides from “Yes, how about you?” Or do you just ask it out of habit or just to show you are nice? Do you dig deep to see if the person is really ok? Why do you ask people “How are you?”

I ask 'how are you' and mean it, but most people are not willing to say the truth and just say 'Fine'. If i ask just to be nice, or acknowledge someone, it turns more into a grunt form of 'alright?' rather than 'how are you?' :P
I ask and then allow people to answer any way they want. If they say "fine" then that's good enough. If they want to talk about how they are feeling, I'm happy to listen.
i just say it to make a bit of conversation,what i don't want is someone that says oh I'm not very good at the moment then proceeds to tell you all there problems,i then have to make a sharp exit.
I leave it alone unless I hear them say "actually" then I always scream as loud as I can SHUT UP TALKIN' TO ME. Is that deep?
