I think there was alot of bitterness left over after the Bush family attacked Cindy McCain in a scandal over stolen painkillers from a international charity hospital. They called her a drug addict. She formally went to the press at that time and she admitted it citing back pain and stress and that she had become addicted. This was when McCain ran his first presidential campaign and I think that attack is the worst the Bush's could have done to him because it was personal and against his wife.
There is no real rift. McCain has voted with Bush 90 percent of the time this year alone. He just tries to pretend he hates Bush because Bush's popularity rating is so low, and he doesn't want to be associated with Bush by the voters.
The fact that McCain had a better chance of running 4-8 years ago and, thanks to Bush, and McCain's support of his failed policies, he doesn't stand a change now.
Bush is a true Republican while McCain is a Republican In Name Only. If you look at McCain's voting record he votes against his own party more than he votes with them, regardless of the DNC talking points.

The 2000 election. The attacks coming from the Bush campaign against McCain were pretty brutal. They also conflicted over the war.
