Spanish is taught commonly enough in US high schools and colleges that many (but not all) people can differentiate between Castilian and Latin American Spanish. I would guess that a third of the people I know could tell the difference because they had at least a basic introduction to Spanish in school. I think that the Spanish spoken in Costa Rica is probably the easiest for Americans to understand, since it is spoken relatively slowly. I almost never have trouble understanding Spanish in Latin America, but I had a lot of trouble in Spain last spring because I had never heard Spanish spoken like that before!
I don't think that Americans who haven't learned Spanish in school can tell the difference. Almost everyone knows that the difference is that Castilian has a lisp, but I don't think many people could actually identify it unless they had taken a few Spanish classes. I know my boyfriend couldn't tell the difference until I started teaching him Spanish!
