You take the puppy outside every 2 hours on a leash to the area you want them to go and do nothing but encourage them to potty. Reward immediately with a small treat when the pup pees or poops. Do NOT use the stupid pads in the house - only tells the puppy its ok to use your house (any where) for its potty area.
Fact is at only 4 weeks, you can't. When we had litters, we eventually extended the original 4 X 4 foot whelping box with another 4 X 4 section, making a big 8 X 4 section. Once that happened, the puppies would take themselves to the front of the box to eliminate instinctively, once up on their legs. Dogs don't usually empty where they sleep, even at that tender age.
Ignoring the why do you have a puppy this young (but why do you??) when it should be with it's mum, siblings and breeder until well onto solids (by around 6 weeks), best you can do at this age is provide an area where it can get to to empty, and hope it does. I don't even think it's right to take a puppy of this age outside. He should be kept snug and warm with mum and siblings, for quite a number of weeks yet - in fact until 8 weeks AT LEAST.

You need to get the puppy to a Vet for a wellness exam especially since it is way too young to be away from its mother. You also need to get it started on the series of puppy shots to protect it from deadly puppyhood diseases. You cannot take your puppy out of your home except for Vet visits until he/she has completed the puppy shots. The puppy will be about 16 to 17 weeks old before you can take it out into the public.
This is a NEONATAL puppy who is incapable of "learning anything" at this age, so you can't do it. It is only operating on "instincts" at this age. That is one of the reasons WHY it is illegal to sell puppies before 7-8 weeks (when they become mature enough to begin to learn). It has NO BUSINESS not being with its mother & litter until it is 8 weeks old.
Whether sold or given to you - the action was VERY ILLEGAL (there are LAWS to prevent the TRANSFER of a puppy of this age) and they are for the PROTECTION of both you & the puppy. It needs to go back to the Backyard Greeder who sold it to you and/or they need to be REPORTED.A puppy this age CANNOT (yet) be given any vaccinations (they will fight against the mother's antibodies in its body) but it is still at HIGH risk for contracting Pavovirus or Distemper - both which have up to 80% fatality rate if contracted.
Give the puppy back to the place he started so he has his mother dog. She will show him the things he needs to KNOW RIGHT NOW. This pup is way too young to be away from his Mom dog.
Once he is closer to about 8 or 10 weeks of age, he will be easier to train. Then, you can look up how do do this. You are going to have many problems with a pup of this age. Either way - DO NOT try to train a pup of this age: Keep in mind that only one week ago this pup had his mother cleaning his butt to make him pee and poop. There is no way he can learn to potty anywhere but the place he is currently standing. He is an infant, perhaps about like a 4 to 6 month old human baby - who by the way would also needs its Mom.
A just over one month old pup is not capable of absorbing stuff like where its supposed to pee an poop and is hardly able to have any control over when and where it does it. A month old pup should still be with its mother and litter mates.
If you can`t take this pup back to its mother, then wait another 2 weeks before expecting the pup to `learn` not to relieve itself indoors.
A good breeder would never allow a pup of 4 weeks old to be taken from it`s mother.
8 weeks is the usual age a pup is sold.

I can only comment on the very few facts you have posted.
You grab the puppy, gently, and take him outside every 20 minutes or so, before he needs to go, take him outside to the designated spot, put him down and tell him "Do your business". Don't take him inside until he is done. If 10 minutes go by and he still hasn't gone, take him back inside and try again 10 minutes later. After he goes, tell him (in a very happy voice) "good boy/girl, what a good dog" - take him back inside and give him a treat (half a doggie biscuit or a cooked chicken heart). Repeat, repeat and repeat. After a short time, you should be able to figure out how often the dog needs to go. Get him outside before he goes in the house/kitchen. Keep his treats handy.
