My son is 5 I have requested an iep I have gone but made the mistake of signing a paper stating that the school could have access to my child’s and mines medical records and now I need to figure out how to take back what I signed as well as I was sent home papers for a full evaluation on my son I was thrilled but when I read what they wanted me to sign I didn’t feel comfortable... why would they want me to sign where it says I understand and give my consent for a full and individual evaluation of me/???? And my child? Please any advice would be great!

Not sure how they would be able to know what doctor you are with to get your medical records. It you can clap your doctors office and explain you do not want your medical records release.
I would write a letter you resend your permission. If possible I would go in to the school/school districts office to deliver this and ask for written verification they acknowledge it.
They may be able to give you back the original paperwork. Ask fo an explanation of why the requested this info on you in the first place.
I would also talk to your son’s doctor
My first thought was to advise you to get an attorney, but others have been wiser than I, and have recommended that you go to the school administrators first. Lawyers are expensive, and if the school officials are fair and honest, they'll simply return the paper to you, and that will be the end of it. You'll need a lawyer only if the school managers won't cooperate.
What did you say when you requested the IEP? Are they looking for a genetic indication that your son will have problems? Contact the school and ask them why they need this information. I have never heard of a school asking for the parent's medical records.
Send them a letter rescinding the permission that you granted. Cite medical privacy laws. Make sure that you get some verification that they got the letter, like return certification etc.
now you need an attorney to get that document back!
Schools have no need for medical records,much less yours, I wouldn't sign that.
Medical records are confidential, Need to know.
We don't know why the school would want that.
I would talk with the school and see if it is a misprint.
