Dont worry it take time one of my cats were in labor for 7 hours just dont crowd her and have towels and when she gives birth to the kittens let her lick them i cant stress that enough.
Emergency vet. If a cat is in major labor with contractions for more then 2 hrs without results then its an emergency and the cat may need a c-section. Likely a kitten is stuck and if its not removed, the rest of the kittens will end up dying inside the mother.
YES, be concerned - although my knowledge is with bitches and puppies, I'd never leave any of my bitches having strong contractions, pushing hard and getting nowhere, for much over 1 HOUR. Every delivery too. All this 'let nature take it's course' could lead to death. If the queen has a big kitten, awkwardly presented, she may well need expert help, even a C.Section and it's way better to get her seen by an expert person, than allow her to become exhausted and then need surgery! In other words better to be safe, than sorry.
