The USUAL practice is to sign the retainer agreement ("hire" the attorney) and then the attorney subtracts his fees from that retainer (dollar amount), bills you monthly. When you run out of retainer money, then the attorney will begin to bill you monthly, and you are expected to pay monthly.
The except is a contingency, and that means that if YOU collect the attorney collects a portion of that money, usually between 25% and 40% (depending on whether or not there's a trial).

Most lawyers ask for a certain amount or a retainer fee unless you and they agree to a percentage of a settlement in a lawsuit as payment.
