It's a good idea to know your blood type and other pertinent information about your health. Ask for copies of your current records after any specialized testing.
They had my blood type wrong, the funniest one is My prostrate is normal. Then 6 years later I have erectile dysfunction. I am a female, yet my first name is predominantly a male name. The VA health records are not always accurate, so if a doc says it's in my records I ask him about my nonexistent prostate.

Yes,I do believe they're accurate.
I know they are not. Especially when they updated to electronic ones, they only scanned three years in. Left a lot of info out. That is not the most troubling part. The sudden addition of about ten opioid based drugs that appeared on my back doc records. That twice I have tried to have corrected and they still haven't been removed. Is troubling. One becouse I do not take any addictive substances, I can't, I had an issue decades ago so I avoid and potential issues. Having them on there makes it appear that it is OK to prescribe them or that I'm faking pain to get them. Two would be they are controlled, I could understand a mistake but not correcting it makes me wonder, if the docs office, is covering for a shortage or something. Which makes me question those that have access and is this the right docs for me. After all, they shove needles into nerves and such.
Probably not, but I don't blame the doctors or nurses. They all kept very good records until laptops replaced clipboards.
Apple and Google would be wise to make their own EHR. Literally anything is better than what is currently out there.
If your point is mistakes happen and it is important to verify the information, it is well taken. I agree.
My weight will be wrong because I dropped a few pounds since my last checkup.
Usually they are very accurate. As you have found firsthand though, mistakes do happen on rare occasions. Sounds like they may have mixed up your file with another patient.
Yes, they are............
I haven't seen a doctor for years nor had hospital visits
