Noones thinking about peeing when bullets are flying. The body suppresses that urge due to adrenaline and movement. It's similar to how the body prevents you from peeing on yourself while you're asleep. You're also sweating more so you're losing liquid through your skin instead of only the bladder. Your body has it's ways of managing all of your bodily functions in different circumstances. Soldiers are also encouraged to use salt tablets on order to make the most retain water during hot weather. And most soldiers take every opportunity to take a quick piss. And the bladder can't relax to release urine while you're under stress and the body is tensed unless you haven't peed for days. And military meals are scientifically prepared to make sure soldiers have what the body needs to perform the job including salt. And even in battle (unlike the movies) there are lulls and timeouts and long waits. Most firefights are actually very short. Soldiers spend a lot of time hurrying to get somewhere and then just waiting to the point they beg for some action so there's plenty of time to pee.
