They can do whatever they want on the store level...since they are independently owned franchises. Some will promote from within while others will not.
They can do what they want but few years ago my nephew applied for management job in McDs and got the job as he had previous experience but never worked in McDs, he left after a few years as he was working long hours for little money.
They can & do both. If someone works there that they think would be a great manager they can promote from within. Or they can choose someone who doesn't work there, but has been a manager at another resturant.

often from within, in their restaurants, and people work their way up. Not always though.
