Not replacing the air filter isn't grounds to fail the emissions test, but if can effect the car's emissions.
Certainly possible but highly unlikely. The filters would have to be extremely dirty or clogged to cause failure. The failure is most likely due to another more significant problem.
I guess if it's clogged enough to cause the MIL to come on for being too rich.
